Medical Records Requests
There is no fee for Immunization Records. Abbreviated medical records are $5 and full charts are $15. A discount will be given for the records of each additional child. Please allow 2 weeks for completion.
School & Camp Forms
- Please ensure the parent/patient section of your forms are properly filled out (if applicable). We need to review this information prior to signing the remaining medical portion of the form.
- We will need a documented well visit in the 12 months prior to your form submission in order to complete your forms. Some forms require a visit within a shorter interval of time. Please check the requirements and schedule an appointment if needed to ensure proper medical clearance.
- Please allow 5-7 business days for your forms to be completed. We are usually able to complete one routine form at the time of your well visit. Additional forms (or those brought in during an acute care visit) require extra time and we will call you to let you know when they are completed and ready for pick up.
- There is a $10 fee for school and camp forms. This is to cover the administrative and provider time required to review your child’s medical history and properly complete the forms.
Maryland School Forms
Prior to your appointment, school forms can be printed from your school's website. Filling them out beforehand will save you time by not having to complete them during your appointment. If you elect not to fill out these forms ahead of time, please arrive 15 minutes prior to your scheduled appointment.
